内容 当教室で直接管理した胞状奇胎(以下奇胎と略)後症例155例を対象とし, 初診時血中human chorionic gonadotropin (hCG), Schwangerschaftsprotein 1 (SP_1)を測定し, 続発症予知の可能性について以下検討した。(1)155例中25例(16.1%)に続発症を認めた。(2)奇胎掻爬前症例19例の血中hCG, SP_1値は続発例では高く, SP_1/hCG比は逆に続発例では低位を示した。(3)奇胎掻爬後症例でも同様に続発例ではSP_1/hCG比が有意に低下した。(4)来院時血中SP_1値と続発例との関係を検討すると, 続発症例の94.9%(18/19)はSP_1の予測減衰曲線を上回っていた。(5)奇胎掻爬後5日で血中SP_1値1.000ng/ml, 10日で100ng/mlをチェックポイントとした場合各々62.5%(5/8), 36%(9/28)が続発症を示し, SP_1値による奇胎後続発症早期予知の可能性が示唆された。
In the 155 postmolar patients, serum levels of human chorionic gonadotropin (hCG) and Schwangerschaftsprotein 1 (SP_1) were simultaneously measured on their first visit and thereafter serially to study the practical means for early prediction of the development of persistent trophoblastic disease (PTD). The following results were obtained. (1) During the course of follow-up (at least 1 year), twenty five of the 155 postmolar patients (16.1%) developed PTD. (2) In the 19 patients with mole in the uterus, serum hCG and SP_1 levels were high in the group with PTD, whereas the ratio of serum SP_1/hCG levels were low in the group with PTD. (3) The ratio of serum SP_1/hCG levels with postmolar patients were significantly low in the group with PTD. (4) Although the serum SP_1 levels in the postmolar patients declined gradually, the SP_1 levels were plotted above the estimated declining line in 18 of 19 patients with PTD (94.9%). (5) Serum SP_1 values were over 1, 000ng/ml (on day 5 after molar evacuation) and 100ng/ml (on day 10) in 8 and 28 postmolar patients, respectively. The occurrence rate of PTD reached 62.5-0x1fd08p+0nd 36.0-1228203520n those two groups. In conclusion, the precise occurrence rate of PTD was 16.1%, and the additional measurement of serum SP_1 levels may be useful for predicting the development of PTD at the early stage of postmolar course.
